If you've already started the Number One Sciatica Exercise for disc bulges and herniated discs, the next step is learning how to safely progress the movement as your symptoms improve. In this section of the Bob and Brad Sciatica Program, we explain how to advance from simple extension exercises to full press-ups while monitoring your symptoms for signs of healing.
Why Progression Matters
The goal of these exercises is not simply to move more—it's to encourage the disc to move away from irritated nerves and promote a process called centralization.
Centralization occurs when symptoms begin retreating from the foot, calf, or leg and move upward toward the low back. Although discomfort in the back may temporarily increase, improving symptoms farther down the leg is often considered a positive sign.
Look for improvements such as:
Less pain in the foot
Reduced numbness or tingling in the leg
Symptoms moving closer to the spine
Increased mobility with less discomfort
Progress only if symptoms remain the same or improve.
Step 1: Baby Press-Ups
Once prone lying and prone prop positions feel comfortable, the next stage is baby press-ups.
To perform the exercise:
Lie face down on the floor or bed.
Place your hands directly beneath your shoulders.
Gently press upward only partway.
Keep your pelvis and hips resting on the floor.
Allow your low back to arch naturally.
Return to the starting position.
Many people with disc problems have significant stiffness and may only be able to lift halfway before the pelvis begins rising off the surface. That's perfectly normal.
Key Guidelines
Perform 5–10 repetitions
Repeat 5–8 times per day if tolerated
Stop if symptoms significantly worsen
Mild stretching sensations are acceptable
Avoid forcing yourself into pain
The goal is gradual improvement in mobility, not pushing through discomfort.
Step 2: Full Press-Ups
As flexibility improves, begin working toward full press-ups.
During a full press-up:
Straighten the elbows as much as possible
Keep the pelvis firmly on the floor
Allow the lower back to sag gently
Only move within a comfortable range
Not everyone will achieve a complete press-up, but it remains an appropriate long-term goal.
Correct Technique
✔ Pelvis stays down
✔ Low back forms an arch
✔ Symptoms improve or centralize
Incorrect Technique
✘ Pelvis lifts off the floor
✘ Movement causes significant pain
✘ Symptoms spread farther down the leg
Remember that healing often happens gradually. Sciatica recovery commonly follows a pattern of two or three steps forward and one step back.
Step 3: Press-Ups with Overpressure
If progress stalls or you cannot reach a full press-up, Bob and Brad recommend adding overpressure.
How to Use a Sheet or Belt
Roll a sheet lengthwise.
Wrap it around the pelvis.
Hold both ends while placing your hands under your shoulders.
Perform the press-up while gently pulling downward on the sheet.
This provides additional stabilization to the pelvis and may help improve extension mobility.
Other options include:
Stretch-out straps
Exercise belts
Assistance from a family member or caregiver
As mobility improves, the extra support can often be removed.
How Often Should You Do These Exercises?
Bob and Brad recommend:
5–10 repetitions per session
5–8 sessions per day when possible
Although this may seem frequent, sciatica can become a chronic issue and may even lead to surgery in some cases. Consistency gives your body the best chance to heal naturally.
Even if symptoms fluctuate, don't become discouraged. Temporary setbacks are common and often occur after prolonged sitting, bending, or poor posture. In most cases, symptoms do not return to their original severity.
