Total Hip Replacement - Exercises 6-9 Weeks After Surgery

Physical Therapist Assistants Mike and Alex demonstrate exercises to progress strength in your legs for 6 to 9 weeks after a total hip replacement surgery. Be sure to check with your doctor about your hip precautions and whether they remain.

Total Hip Replacement Exercises: 6–9 Weeks After Surgery

At 6–9 weeks after total hip replacement surgery, many people are ready to begin progressing their strength, balance, endurance, and everyday mobility. In this phase of the Total Hip Replacement Fitness Program, physical therapist assistants Alex and Mike demonstrate exercises designed to help you safely continue your recovery while gradually returning to normal activities.


Before advancing your exercises, check with your surgeon or healthcare provider about your specific hip replacement precautions and range-of-motion restrictions. Everyone heals at a different pace, and some precautions may remain in place beyond six weeks.


The main goals during this stage are to progress hip strength, improve balance, increase walking endurance, and gradually add resistance. Exercises may include standing hip exercises with cuff weights, mini squats, heel and toe raises, stationary biking with added resistance, progressive walking, wall slides, and single-leg balance exercises.


Start with approximately one set of 10 repetitions and gradually increase repetitions, resistance, or sets as tolerated. For cardiovascular exercise, work toward 20–30 minutes on a stationary bike and progressively increase your walking distance. If you still need a cane or other assistive device for safety or longer walks, continue using it as needed.

Pain, increased limping, or significant soreness can be signs that you are progressing too quickly. Let your symptoms guide you and adjust your activity accordingly.
