How to Sleep Pain-Free With Back Pain: 7 Tips That Actually Work

Back pain is exhausting—literally. When you're dealing with chronic back pain, a restless night only makes the next day worse. Your body desperately needs quality sleep to heal, but back pain makes that nearly impossible. The good news? There are proven strategies to help you sleep better, even when your back is hurting.

We've put together seven tested methods to help you get the pain-free sleep you deserve. The key is experimenting to find what works for your unique body.


1. Use a Leg Wedge (or Stack Pillows If You're on a Budget)

This is Brad's favorite for good reason. A leg wedge elevates your legs in a way that naturally aligns your spine and relieves pressure on your back.

Here's the setup:

  • Place the leg wedge under your legs

  • Add a cold gel pack (the soft kind that stays pliable when frozen) directly on your sore spot

  • Toss a pillow behind your back for support

  • Relax for at least 15 minutes

Brad's go-to move? Sleep with this setup for about two hours while the cold pack does its job. Once your back settles down, remove the pack and sleep normally for the rest of the night.

Budget option: Stack three pillows under one leg for a similar effect. It won't feel identical, but it's surprisingly effective if you're not ready to invest in a wedge yet. If you find yourself using it regularly, that's when it's worth opening your wallet.


2. Add Lumbar Support With a Rolled Sheet (or a McKenzie Sleep Roll)

If you have curves—a bigger buttock, fuller hips, or a pronounced lower back curve—this one's for you.

The gap between your lower back and the bed causes stress and pain. A simple rolled sheet tied around your waist fills that gap perfectly, whether you're lying on your back or sleeping on your side.

DIY method:

  • Take a long sheet and tie it around your waist

  • Lie down and notice how it supports your lumbar spine

  • Works equally well on your back or side

Upgrade option: Invest in a McKenzie Sleep Roll, designed specifically for this purpose. It has tie wraps to match your body size and waist, giving you the same support with extra comfort. If the sheet works well, you don't need to buy one—especially if this is temporary pain relief.


3. Sleep on Your Back With Pillows Under Your Legs

Simple, effective, and requires nothing you don't already have.

Sleeping on your back with one, two, or even three pillows underneath your legs takes pressure off your lower back and promotes alignment. Adjust the number of pillows based on what feels most comfortable to you.


4. Place a Pillow Between Your Knees When Sleeping on Your Side

Here's what happens: a pillow between your knees stops your spine from rotating and reduces hip stress while you sleep.

This is one we actually do every night. The relief is noticeable. It takes the rotational strain off your spine and keeps your hips happy too.


5. Sleep in a Recliner (If You Have To)

Some people have no choice—they can only sleep in a recliner. If that's you, make it work.

Recliner sleeping tips:

  • Recline the chair as far back as it will go while staying comfortable

  • Place a lumbar support pillow (or smaller throw pillow) behind your lower back

  • Feel the difference it makes immediately

You might also try the leg wedge on a regular bed first—it could replicate what the recliner is giving you, but with a lot more comfort.


6. Try the 90-90 Position

This one's as much a nap position as a full sleep solution, but it works.

Lie on your floor (yes, really), and put your feet up on an ottoman, stool, or chair. Your hips bend at 90 degrees, and your knees bend at 90 degrees. It's surprisingly comfortable for short periods, and you can add a cold pack underneath for extra relief.

Important: Don't sleep like this all night on a hard floor. Use a mattress topper if you plan to extend the time, or just enjoy it as a restorative nap.


7. Fix Your Mattress

Sometimes the problem isn't your position—it's your bed.

If your mattress is sagging, it's working against you. Try one of these solutions:

  • Put the mattress on the floor for a firmer, more supportive sleep surface

  • Add a mattress topper for cushioning and support

  • Put plywood between the mattress and box spring to firm everything up

Any of these will give you the solid support your back needs to actually heal while you sleep.


The Bottom Line

Good sleep isn't a luxury when you have back pain—it's essential medicine. Your body does its healing work at night, so investing in better sleep positions and support is investing in your recovery.

Try these methods one at a time and pay attention to what feels best. Your back will tell you what it needs. And remember: getting those Z's might be the best thing you do for your pain management today.
