How to Strengthen Your Core and Fix Lower Back Pain: 3 Proven Exercises from the Experts

If you're dealing with lower back pain, you're not alone. But here's the good news: strengthening your core might be exactly what you need. Bob and Brad, two of the internet's most trusted physical therapists, break down the science and the solution in three simple moves.


Why Your Core Matters for Back Pain Relief

Your abdominal muscles aren't just for show. They're actually designed to work like a natural corset—a protective belt around your spine that supports and stabilizes your back.

Here's how it works: The muscles in your core (the rectus abdominis, external obliques, internal obliques, and transversus abdominis) connect deep into the fascia of your back. When you engage these muscles, they tighten around your torso and meld into the connective tissue that protects your spine. It's a sophisticated system, and when your core is weak, your back pays the price.

The transversus abdominis—your deepest abdominal muscle—wraps horizontally around your torso like a weight belt. The obliques run diagonally and at right angles to each other, creating a web of support. And the glutes and latissimus dorsi muscles also tie into this system. When all these muscles are working together, they create stability that your back desperately needs.


The 3 Essential Core Exercises

Dr. Stuart McGill, a world-renowned back pain expert, recommends these three exercises as the foundation for core strength. They're simple, proven, and you can do them at home.


Exercise #1: The Curl Up

What it does: Targets the front abdominal muscles while protecting your neck and back.

How to do it:

  1. Lie on your back on a firm surface (floor or bed)

  2. Place both hands underneath the small of your back, supporting the natural curve

  3. Keep your head aligned with your shoulders—don't flex your neck

  4. Lift your shoulder blades off the floor by raising your elbows (think of pointing your elbows down and back)

  5. Your head and shoulders rise together toward the ceiling

  6. Hold for 10 seconds

  7. Rest and repeat 6–10 times

Pro tip: You're not doing a full sit-up. Keep the movement small and controlled. If you want more intensity, lift your elbows up instead of letting them touch the floor—this isolates the muscle even better.


Exercise #2: The Side Plank

What it does: Strengthens the obliques and the deep core muscles that stabilize your spine from the sides.

Beginner version:

  1. Lie on your side with knees bent

  2. Line up your knees and ankles

  3. Push up onto your knees, keeping your body in a straight line

  4. Hold for 10 seconds

  5. Repeat 5–10 times on each side

Advanced version:

  1. From the same starting position, straighten your legs

  2. Push up onto your feet instead of your knees

  3. Hold for 10 seconds

  4. Repeat 5–10 times on each side

Important: This one is challenging—there's a significant jump in difficulty from knees to feet. Start with the beginner version and work your way up. Do the movement slowly with smooth, controlled breathing.


Exercise #3: The Bird Dog

What it does: Strengthens deep core muscles while improving balance and coordination. It targets the muscles that directly support your back.

Beginner version:

  1. Start on all fours (quadruped position)

  2. Keep your back straight across, like a table

  3. Slowly extend one arm forward, pointing like a bird dog points at game

  4. Hold for 10 seconds

  5. Return and repeat with the other arm

  6. Then raise one leg at a time, holding for 10 seconds each

Advanced version:

  1. From all fours with a straight back

  2. Simultaneously extend your right arm and left leg, stretching them as straight as possible

  3. Hold for 5–10 seconds

  4. Return to center

  5. Repeat with left arm and right leg

  6. Complete 10 cycles on each side

Pro tip: Do this on a carpeted floor, not a bed—beds are too unstable. You'll notice wobbling as you get stronger, which actually means the exercise is working even harder to challenge your balance.


The Bottom Line

These three exercises—the curl up, side plank, and bird dog—form a complete core-strengthening routine backed by science and trusted by physical therapists. You don't need fancy equipment or hours at the gym. Simple, consistent effort is what builds a stronger, pain-free back.

Start with 5–6 repetitions of each exercise, holding for 10 seconds per rep. As you get stronger, work up to 10 repetitions. Do them regularly, maintain good form, and you'll notice the difference.
