How to Massage the IT Band for Knee Pain Relief


Relieve IT Band Syndrome and Improve Mobility

Pain on the outside of the knee is often caused by Iliotibial (IT) Band Syndrome, a common overuse injury seen in runners, cyclists, and active adults. In this program, physical therapists Bob and Brad explain how the IT band works, why it becomes painful, and how massage can help reduce discomfort and improve movement.


The IT band is a thick band of connective tissue that runs from the hip to the outside of the knee. Because it is extremely strong, it cannot be significantly stretched. Instead, massage helps improve tissue mobility and reduce adhesions that may develop between the IT band and surrounding structures.


A key area to address is the Tensor Fascia Lata (TFL), a muscle on the front and side of the hip that attaches directly to the IT band. Tightness in the TFL and surrounding hip muscles can contribute to IT band tension, outer knee pain, anterior pelvic tilt, and altered movement patterns.


Using a massage gun, first target the TFL and gluteal muscles before working along the length of the IT band. Move slowly, focus on tender areas, and avoid aggressive pressure. This can help improve mobility, reduce muscle tightness, and relieve symptoms associated with IT Band Syndrome.


Common Symptoms of IT Band Syndrome

  • Pain on the outside of the knee

  • Burning or aching during running or biking

  • Clicking, popping, or snapping sensations

  • Pain that may travel toward the hip

  • Tenderness along the outer thigh
