Who should be responsible for organizing transportation after a hospital stay

When my partner was discharged after a short hospital stay, everyone kind of assumed someone else was handling the ride home. The nurse mentioned discharge, the doctor moved on, and suddenly we were standing there realizing no one had actually confirmed who was picking him up or what kind of help he’d need getting home. I’ve seen this play out a few times, and honestly it works best when one person is clearly “the coordinator,” even if others help. With my mom, I took that role and stayed in touch with both the care team and family. The hospital gave general guidance, but they don’t really manage the logistics for you. I found some useful reminders while reading tips for safe hospital-to-home travel, especially about asking questions early and confirming details instead of assuming. Once someone owns the plan, it’s easier for everyone else to plug in and help without confusion.
