Do You Have Sciatica? 4 Simple Self-Tests to Help Identify the Cause of Your Leg Pain

Sciatica is one of the most common causes of low back pain that radiates into the buttocks, thigh, or leg. However, not all leg pain is true sciatica. Conditions such as a herniated disc, spinal stenosis, and piriformis syndrome can produce similar symptoms, making proper diagnosis essential.

In this lesson from the Bob and Brad Sciatica Program, physical therapists Bob Schrupp and Brad Heineck demonstrate four simple self-tests that may help you better understand what could be causing your symptoms before speaking with your healthcare provider.

Test #1: Seated Straight Leg Raise for Sciatica

The seated straight leg raise test is designed to identify possible irritation of the sciatic nerve caused by a herniated disc or bulging disc.

While seated, straighten one knee, pull your toes toward you, and gently bend your neck forward. If this movement reproduces pain, tingling, or symptoms that travel down your leg, it may indicate nerve irritation associated with sciatica.

Be sure to compare both sides, as a significant difference between legs can provide valuable information.


Test #2: Spinal Compression Test for Disc Problems

This second self-test places gentle pressure on the spine to determine whether a bulging disc or herniated disc may be contributing to your symptoms.

While seated in a slouched position, grasp the underside of a chair and pull upward. Then move your neck forward and backward while noting any changes in symptoms.

If bending your neck forward increases pain that travels into the leg, a disc-related problem may be involved. If simply sitting slouched immediately worsens your symptoms, stop the test and consult a healthcare professional.


Test #3: Three-Part Screening for Spinal Stenosis

Not all sciatica-like symptoms are caused by a disc problem. In adults over age 55, lumbar spinal stenosis is another common cause of leg pain.

This screening includes:

  • Bending forward while seated to see if symptoms improve.

  • Standing and leaning backward to see if symptoms worsen.

  • Answering three key questions:

    • Does prolonged walking or standing increase your pain?

    • Does sitting or leaning on a shopping cart reduce your symptoms?

    • Are you over 55 years old?

If you answer "yes" to all three questions and your symptoms improve when bending forward, spinal stenosis may be a contributing factor.


Test #4: Piriformis Syndrome Test

Sometimes referred to as "false sciatica," piriformis syndrome occurs when the piriformis muscle in the buttocks irritates the sciatic nerve.

To test for piriformis involvement, place the ankle of the painful leg across the opposite knee and lean forward. You may also perform a lying stretch by bringing the knee toward the opposite shoulder.

If one side feels significantly tighter, reproduces buttock pain, or causes symptoms down the leg compared to the other side, the piriformis muscle may be contributing to your discomfort.


Why Proper Diagnosis Matters

Sciatica symptoms can result from several different conditions, including:

  • Herniated discs

  • Bulging discs

  • Lumbar spinal stenosis

  • Piriformis syndrome

  • Other nerve-related conditions

Because treatment varies depending on the underlying cause, identifying the source of your symptoms is critical. The Bob and Brad self-tests can help you gather useful information, but they should never replace a professional evaluation.
